Enrolling in our center ? These forms below will help expedite the process.
All forms are required by the State of Wisconsin, please download each form - fill out- and return prior to your 1st day starting along with the registration fee.
Please note there will also be extra forms that will also need to be filled out for our center before your child can attend our program. These forms are specific to our center, and are only available for pick up at our front office. These forms are handed out to families who have paid the registration fee and will be enrolling in our center.
